Why might anyone in the Arabic speaking world be concerned about the “blood sugar roller coaster”? Blood sugar goes up and down, sometimes too high (hyperglycemia) and sometimes too low (hypoglycemia). What does this fluctuation in blood sugar have to do with your health, and especially your metabolic health? And, you might even ask, “What is metabolic health?”

“Metabolically healthy” can be defined with standard indicators such as having normal levels of blood sugar, triglycerides, high-density lipoprotein (HDL) cholesterol, blood pressure, and waist circumference, without using medications. However, there are pathways that lead to metabolic disorders that we should pay attention to much earlier than when these medical markers manifest or are analyzed later in life (One of our medical advisors, Dr. Robert Lustig talks about these pathways in his book, Metabolical). Many metabolic factors comprise a person’s risk profile for developing a leading chronic disease.

Too many focus on obesity (body weight), but we don’t die of “obesity.” We die of the diseases that “travel” with it or drive us to it – metabolic derangements that are often associated with obesity, but that also underlie metabolic disorders affecting the non-obese, such as non-alcoholic fatty liver disease. Diabetes, hypertension, heart disease, cancer, liver disease, and dementia are the chronic diseases that kill us – but underneath most of these conditions is almost always some form of metabolic dis-ease, and the runway is long, sometimes going all the way back to the womb. Starting off life metabolically healthy, especially in the first five years, can have positive outcomes lasting a lifetime. Nonalcoholic fatty liver disease (NAFLD), once mainly associated with adult alcoholics, is now sadly emerging as one of the most common liver diseases in children, with an increasing prevalence that is alarming both families and their doctors. The modern diet, filled with processed carbohydrates and sugar, is literally turning out children’s livers into “Foie gras”.

Several critical factors are converging that concern all of us:

  • a global tsunami of billions of people suffering from life-altering, chronic, preventable
  • diseases such as diabetes, heart disease, and cancer
  • increased vulnerability to acute diseases such as COVID-19, and emerging health issues that are eroding the human immune system
  • a lack of improvement in global human and environmental health
  • growing disparities in the access to nutritious, affordable food

But what about the Arabic-speaking world?

“Diabetes mellitus (DM) affects more than 382 million people around the world, of whom 90% are diagnosed with type-2 DM (T2DM)[1]. The prevalence of T2DM has increased dramatically during the last 2 decades[1]. The Arab world is not immune from this epidemic in the prevalence of T2DM. In fact, the Middle Eastern and North African region has the second-highest rate of increase in diabetes globally, with the number of people with diabetes is projected to increase by 96.2% in 2035[1].

Fifty-five million adults (20-79) are living with diabetes in the Middle East North Africa (MENA) Region in 2019. This figure is estimated to increase to 108 million by 2045. Metabolic syndrome, a combination of medical disorders that, in concert, increase the risk of developing cardiovascular disease and diabetes. This affects about one in four people in the Middle East.

So, what does this have to do with your blood sugar?

Blood sugar dysregulation (what we call the “blood sugar roller coaster) is one “opportunity” for you to intervene early before progressing to diabetes or other forms of metabolic disease. Blood sugar issues (both high and low) are indicators (“canaries in the coal mine”) that can help you pay attention to health issues that might affect your long term health, and even prevent the progression of chronic metabolic disorders.

Why is hypoglycemia the elephant in the room?

The Blind Men and the Elephant is an ancient parable that lays bare overreaching misinterpretations. The moral of the parable is that “experts” may lay claim to absolute truths based on their subjective experience while ignoring contradictory evidence. This fable holds so true when the subject of hypoglycemia comes forward either in a conversation or the written word. Most likely this occurs because there are many types of hypoglycemia and many causes, so advice often seems confusing and conflicting. What is certain is that hypoglycemia is one of the most perplexing, complicated, misunderstood, and misdiagnosed conditions of the 21st century. It’s not clearly understood by many in the medical profession, who often diagnose and treat patients by addressing symptomatic aspects of hypoglycemia versus the root causes.

“Hypoglycemia occurs most often in diabetes when the doses of insulin or medications which stimulate insulin release are too high relative to food or activity. However, hypoglycemia can occur also when the body is making too much insulin on its own. This can occur due to an insulin-producing tumor (insulinoma), or when the pancreas makes too much insulin after meals due to abnormal function of the insulin-producing cells, some cases of prediabetes, or due to abnormally high levels of hormones which direct the pancreas to make too much insulin. This can occur after a variety of types of gastrointestinal surgery. These conditions are not as common as diabetes, so are less recognized by the general public. However, for those patients affected by hypoglycemia, it is just as dangerous whether associated with diabetes or whether it occurs outside of diabetes.”

In light of the current global pandemic of type 2 diabetes, the perspective of Dr. Seale Harris, the pioneer researcher of hyperinsulinism who viewed hypoglycemia as a harbinger of more complex metabolic disease as early as 1924, was prescient: “The low blood sugar of today is the diabetes of tomorrow.” Today, low blood sugar is an issue that affects the majority of the population in some form or another and is a key indicator that metabolic dysfunction is at play. Patients, adequately informed by their doctors, can take action to address diet and lifestyle factors that are driving the condition before more advanced diet-related disorders progress. Unfortunately, many doctors are poorly prepared to diagnose and treat the condition within the context of metabolic (systemic) dysfunction.
